Referral Form - IV, Welsh Grants or Parent Advocacy Referrals.
Thank you for choosing to make a referral to NYAS. We value each and every referral so that we can make a difference to children, young people and some adults. In order for us to respond quickly to your referral, please take note of the following:
- No referral will be accepted unless there is a minimum of 5 working days’ notice when the reason for the referral relates to a specific meeting.
- All mandatory fields must be completed. If they are not, your referral will not be accepted.
- If there is further incomplete information once the referral has been submitted, we will contact you. Due to the high volume of referrals we will only make contact with you once. If you do not respond to our request, you will need to resubmit the full referral and your original referral will not be actioned.
This system of referral has a high level of information security…. All information entered onto this form and sent to us, is done so through a secure server connection using the Secure Sockets Layer (SSL) protocol. This allows your browser to encode any data it sends to our server, so that even if someone intercepted your information as it travelled across the Internet they wouldn’t be able to read it.
